University Registrar Graduate Student Forms

Student forms submitted to the Registrar's Office or the Student Services Center are now housed in the new Stanford eForms platform.  To go to Stanford eForms, log in to Axess, click on the Student tab, and choose “Student eForms” from the Quick Links menu. Click on the “Available Forms” to find student forms.  Additional information about eforms can be found here:  https://registrar.stanford.edu/resources-and-help/student-forms/graduate-student-forms

Forms to be submitted via Axess eForms:

• Graduate Program Authorization Petition (to add MA degree)  
• Application for Graduate Residency Credit  (used to transfer units from prior MA degree to Stanford)
• Application for Ph.D. Minor
• Request for Terminal Graduate Registration (TGR) Status
• Graduation Quarter Petition (reduces tuition to $150 in final quarter in which you submit dissertation)
• Graduate Petition for Part-time Enrollment (use when only 3-7 units needed for TGR status) *International students consult Bechtel for visa compliance prior to submitting
• Leave of Absence Form

PDF Forms submitted to the Assistant Director of Student Services:

• Application for Candidacy
• Program Proposal for a Master's Degree
• Thesis Advisor Agreement (for 3rd year review)
• Dissertation Reading Committee Form (required for TGR)
